What is the top item?

I'm relatively new to crypto. The other day a friend sent me some USDT erc-20. They arrived just fine, but when looking at etherscan, it appeared as two different tokens/coins/whatever was sent to me. What is the top item, with the weird mirrored S? Is it something, that in any way require my action? I have been sent USDT erc-20 before to the same wallet, but haven't seen this before.
He sent from trustwallet, if that makes a difference.
